As we age, getting the care and support we need to live comfortably and safely becomes more important. That's where Indiana PathWays for Aging comes in — a program designed specifically to help older adults like you stay healthy, independent, and supported.
What is Indiana PathWays for Aging?
Indiana PathWays for Aging is a state-managed Medicaid program that helps eligible seniors (age 60 and older) access the long-term services and supports they need, whether they live at home, with family, or in a care facility.

The program connects you with services like:
Home health care
Meal delivery
Transportation to appointments
Help with bathing, dressing, and medications
Nursing home or assisted living support, if needed
Care coordination to manage all your services
How Can It Help Me Stay at Home?
One of the biggest goals of the PathWays program is to help you stay in your home and community as long as possible, instead of moving into a nursing home before you need to.
With PathWays, you may qualify for in-home support, like a caregiver who visits regularly to help with daily tasks, or someone to help with household chores. You may also get connected to community programs that provide meals, exercise, wellness checks, and more.
Who Will Help Me?
When you enroll, you’re matched with a Care Coordinator. This person is your go-to helper — they’ll talk with you about your health, your home, your daily needs, and work with you to create a personalized care plan.
They will:
Help schedule services
Make sure you’re getting the care you need
Keep in touch to update your care as things change
Who is Eligible?
You may be eligible if:
You are 60 years of age or older
You receive Medicaid
You need help with daily activities like bathing, dressing, cooking, or managing your medications
A professional will complete an assessment to determine what kind of support you qualify for.
